    They could be fleas or other parasites

    That definitely sounds like worms. People tend to think that dogs are wormed after a puppy, there is no need to do it again. A simple mistake, but simply not the case. You should be wormed at least once per year since the worms can be picked up during something as innocent as a daily walk. See your veterinarian, the deworming is very inexpensive. I would start there.
